New patients should arrive 15 minutes prior to appointment to complete registration process.New patients are required to bring a referral if required by your insurance company.
It is a requirement to bring your mammogram films and report on your appointment.
We would appreciate if you would complete the Medical Information Sheet located on the website. Please bring all your medicines. If a previous medical record, EKG, chest x-ray, and/or hospital summary are available; we would appreciate your bringing them with you.
Our office sees patients by appointment only. If you are unable to keep your appointment, please notify us as early as possible, so that we are able to offer the time to another patient. Please take into consideration that we are a surgeons office; therefore, we must recognize that emergencies and unexpected complications do arise and must be accommodated. To schedule or cancel an appointment, please call 281-922-5550. Our staff will gladly assist you in making the most convenient appointment to you that is available.
